Children & Youth Registration

If you are interested in your child attending Sunday School or Youth Ministry activities at Mystic Congregational Church, please complete the registration form below.

We only need one form per family, so please be sure to include all of your children. If you need to register more than 3 children, please contact Aidan at aidan@mysticucc.org.

I understand that during the course of the year, pictures & videos may be taken to help us remember and document our events and accomplishments. I give permission for my child’s picture/video to be used in church publications such as, but not limited to: The Anchor Newsletter, the church Website, church Facebook page, and Children’s Ministry’s Family Notes emails.
